Community Care Labs & Mobile Services, LLC ("we," "us," or "our") provides professional specimen collection and mobile phlebotomy services only. We do not perform laboratory analysis, interpret results, or issue medical diagnoses. All laboratory testing, analysis, and reporting are performed by independent, accredited partner laboratories under established services agreements.

Specimens collected by Community Care Labs are forwarded to one of the following accredited partner laboratories, based on the requested test panel and the requirements of the ordering employer, attorney, court, healthcare provider, or individual:

Each partner laboratory is responsible for the testing methodology, quality control, certified review (where applicable), and reporting of results directly to the authorized recipient.

All regulated and chain-of-custody specimens are collected, sealed, labeled, and documented in strict accordance with applicable federal, state, and industry protocols, including 49 CFR Part 40 for DOT collections. Specimens are transported to the designated partner laboratory in tamper-evident packaging.

We collect only the information necessary to perform your requested service — typically your name, date of birth, contact information, government-issued photo identification (for chain-of-custody collections), the ordering party, and the test(s) requested.

Personal health information is kept confidential and is shared only with: (1) the designated partner laboratory performing the testing, (2) the authorized ordering party (employer, attorney, court, healthcare provider, or you), and (3) other recipients you have specifically authorized in writing or as required by law.

We do not sell, rent, or trade your personal information. Records are retained in accordance with applicable regulatory and contractual requirements.

Information provided by Community Care Labs, including any communications related to scheduling, collection logistics, or general service descriptions, is for informational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment. Test results are released by the partner laboratory directly to the authorized ordering party. Community Care Labs does not release, interpret, or modify results. Questions about a specific result should be directed to the ordering party or the partner laboratory of record.
